Filing a Title IX incident report serves as official notice to Westminster College and authorizes the Title IX Coordinator to initiate a preliminary investigation into the information and allegations contained within the report. To submit this form anonymously, simply exclude any personally identifiable information. Please understand that a preliminary investigation may lead to the identity of the person submitting this form whose name will be kept confidential by the Title IX Coordinator.
While this report serves to notify Westminster College of an alleged Title IX incident, a Formal Complaint is required to engage a formal investigation and grievance process. The Formal Complaint form is available by contacting the Title IX Coordinator.
