Title IX Incident Report


The Title IX Incident Report may be submitted by any person who would like to inform the College that an incident occurred which may constitute sexual harassment, sexual violence, or gender discrimination. This form may be submitted anonymously. If you have questions about how to complete the form, please contact the Student Affairs Office for assistance at (724) 946-7110.

Upon receipt of the Title IX Incident Report, the Title IX Coordinator will contact the appropriate individuals for further information. Filing a Title IX Incident Report does not initiate an investigation or the grievance procedure. The Title IX Coordinator will advise the complainant on reporting options, available supportive measures, resources on and off campus, and how to formally request a grievance proceeding.
