Enrollment health forms are due by July 1 and are submitted through our student health portal located at https://westminster.medicatconnect.com. Health forms are required of all Westminster students to assist our staff in meeting your health care needs.
Prior to July 1, confirm that the required immunizations have been received and obtain an official immunization record from your health care provider or school.
Westminster College requires:
- 2 doses of MMR vaccine
- 2 doses of varicella vaccine
- Tdap vaccine within the past 10 years
- 3 doses of hepatitis B vaccine
- 2 doses of meningococcal ACWY vaccine (second dose after age 16).
Scan or save any documents to your computer or smart phone. Make sure your name and Student ID are on all documents.
Images must be .gif, .tiff, .jpg or .jpeg. Documents must be .txt or .pdf. Files must be smaller than 4 MB and file names cannot include any special characters.
Log in to the student health portal located at https://westminster.medicatconnect.com with your Westminster email and password.
- Navigate to “Forms” in the portal menu and complete and submit the required forms.
- Health history
- Meningococcal acknowledgement and waiver
- TB screening questionnaire
- Navigate to “Immunizations” in the portal menu. Click on each drop down for the required vaccines and enter the dates you received each vaccine.
- If you have not received vaccine(s) or do not have immunization documentation, labwork documenting immunity will be accepted. Click on the “Alternate Documentation” drop down and enter the date the labwork was collected.
- Navigate to “Uploads” and upload documentation of the labwork documenting immunity
- If you are seeking to waive vaccines for religious or philosophical reasons, you must call the Wellness Center at 724-946-7927
- Navigate to “Uploads” and upload a copy of your immunization record
- You may upload an image of your insurance card or other information that you wish to share with the Wellness Center
