Class Absences Due to Illness

The Student Health Center will provide Report of Absence forms as needed. Unless indicated specifically by an SHC staff member, this form is for information purposes only, and not an excuse from a class(es). Excuses for a single absence from a physical education class as a result of injury or illness can be given by the College nurses before the class meets. Only the College doctor can excuse you from a physical education class for more than one class at a time.
