Please complete the following form to reserve Audio Visual equipment. When complete, click SUBMIT at the bottom of the form. Audio Visual staff will review your request, check for equipment availability and email confirmation to the user name provided.

*Equipment reservations require an advance notice of two business days to allow for proper scheduling, staffing, and maintenance. Same day requests and checkout are not possible. Please see our POLICIES page for more information.

In order to connect to the Apple TV, your iDevice must be connected to the Wifi network "WC-Collaborate". This needs to be communicated with Don Goodlin the Telecommunications & Network Engineer. To connect the Apple TV to an Analog System, the Kanex HDMI to VGA/Mini audio converter is required.

By submitting this form, I understand that these requests are reviewed individually by the department of Audio Visual Services in the order they are submitted. I also understand that upon review of availability and current college policy, a determination will be made if AV can meet my needs, at which time I will be notified by e-mail. I understand AV does not provide service or take responsibility for any equipment provided by others.

I understand that I will accept any equipment approved for this request in good working condition with all accessories as listed. I will use said equipment only for the event listed in a legal, responsible, proper, and safe manner. It is my responsibility to know and understand permitted uses and limitations outlined by federal, state and local laws as applicable. This equipment will be under my control at all times. I take full responsibility (financial and otherwise) for replacement, repair, and return of said equipment if it is lost, stolen, damaged, or otherwise misplaced or rendered unusable in any way. This includes any instance wherein another party gains access to said equipment, with or without my knowledge or approval. I will return said equipment by the required return date and time listed in the confirmation email, and will return it only to the Westminster College Audio Visual Department. I understand that there is a time limit on my use of this equipment, and that fines will be charged as posted for any overdue returns. I also understand that I may request a single extension for the return of this equipment prior to the due date, that requesting an extension does not mean it is approved, that AV approval for all extensions is necessary, and that requesting an extension will not absolve me from fines previously accumulated. I hereby certify that I am competent to contract in my own name in so far as the above is concerned. I have read this agreement before affixing my signature below, and warrant that I fully understand and agree to the contents thereof.
